                "valueString" : "<p>As part of your role providing health or disability support services, you’re using your Electronic Medical Record (EMR) or Practice Management System (PMS) to access Health New Zealand (HNZ) services, like the Aotearoa Immunisation Register and Screening Registers. This means you will access and contribute to personal health information (the Information) of individuals receiving care. Privacy is very important to us all, and HNZ, as the guardian of this Information, relies on you to handle it responsibly and only as allowed. Please read the following terms carefully before accessing Information. By proceeding, you confirm that you:</p><p></p><p>Comply with Privacy Laws: Follow the Health Act 1956, Privacy Act 2020, and the Health Information Privacy Code 2020 when handling any Information.</p><p></p><ol><strong><li>Complete Required Training</li></strong><dt>Take any training needed to access the Information. Your employer will inform you about this training.</dt><p></p><strong><li>Inform Patients of Privacy Policies</li></strong><dt>Make sure each person whose information you access has been provided with relevant Privacy Statement materials or can reasonably be expected to have received these from your employer.</dt><p></p><strong><li>Use the Information Appropriately</li></strong><ul><li>Only use the Information for delivering health or disability services to those in your care.</li><li>Keep the Information safe and confidential, like securing it within care records and ensuring it’s not visible on unattended screens or areas.</li><li>Access or download only what you need for your job and share it only with others who need it for theirs. </li><li>Never share your login details or let others use them.</li><li>Don’t let others use your device or computer to access Information unless they use their own credentials. </li><li>Only access Information using secure, approved devices or systems that are password-protected or encrypted and updated with security software.</li></ul><strong><li>Audit and Monitoring</li></strong><dt>Understand that all access to Information will be logged and monitored by Health New Zealand for audit purposes. Individuals have the right to see who accessed their information. You agree to co-operate with HNZ around any access or privacy related queries.</dt><strong><li>Accuracy of Information</li></strong><dt>While HNZ works to provide high-quality data, it might not always be perfect. Using this data does not replace good clinical judgment and quality care.</dt><strong><li>Report Privacy Issues Quickly</li></strong><dt>If you suspect a privacy or security breach, immediately inform both the Te Whatu Ora (HNZ) Privacy Officer (hnzprivacy@TeWhatuOra.govt.nz) and your own Privacy Officer. Reporting quickly helps resolve issues faster. </dt><strong><li>Assist in Breach Investigations</li></strong><dt>Help with any investigation into privacy breaches if requested.</dt><strong><li>Your workplace application’s role</li></strong><dt>Your application might send information to HNZ for national records, like patient details or immunisation records. To ensure this information is accurate:</dt><ul><li>Keep health records current and update them as soon as possible.</li><li>If your workplace uses both paper and electronic records, make sure you follow procedures to keep the electronic record as the main record.</li></ul><strong><li>National Cervical Screening Programme (NCSP)</li></strong><dt>Only authorised people can access NCSP information. Authorised roles include:</dt><ul><li>Registered Healthcare Providers providing cervical screening services</li><li>Those with a specific role approved by the NCSP Manager for outreach or related services</li></ul><dt>If you are authorised to access cervical screening information:</dt><ul><li>Understand that your access is legally controlled by Part 4A of the Health Act 1956 and follow those legal requirements.</li><li>Complete any training requested by the NCSP</li><li>Follow all standard procedures, policies, and quality standards relevant to your role in the NCSP.</li></ul><strong><li>Updates and Non-Compliance</li></strong><dt>These terms may change over time, and you’ll need to accept any updates before accessing Information again. If you don’t comply with these requirements, your access to HNZ services may be removed, and the issue may be reported to your employer or regulatory authority.<p></p>Before continuing, please confirm the details your system has provided are correct.
